HIV + Aging Curriculum

Description
Welcome to the HIV + Aging Curriculum, a comprehensive training designed to equip case managers with the knowledge, skills, and tools to provide affirming, compassionate, and person-centered care for older adults living with HIV. Created by AIDS Foundation Chicago and The Care Plan, this curriculum addresses the unique challenges faced by long-term survivors and older adults aging with HIV, empowering case managers to advocate effectively and foster equity in their care.

Through six thoughtfully crafted modules, participants will explore historical, medical, social, and cultural aspects of HIV and aging, building practical expertise to navigate complex healthcare and social support systems. Each module emphasizes actionable strategies, fostering trust, respect, and dignity in care relationships.

This project is supported by the Administration for Community Living (ACL) and the US Department of Health and Human Services. The HHS is part of a financial assistance award totaling $250,000 with 100% funding by ACL and HHS. The contents are those of the authors and do not necessarily represent the official views of nor are an endorsement by ACL, HHS or the US government.

AIDS Foundation Chicago mobilizes communities to create equity and justice for people living with and vulnerable to HIV or chronic conditions.
